Getting DDR4 memory to run robustly at 4000 can be tricky. My guess is that your rig will stabilize once you disable XMP profile and run the memory at stock JEDEC 2133 speed, or back the speed down...

dmidecode may be reading the "standard" 2133...
dmidecode may be reading the "standard" 2133 timing set from the DDR4 SPD, instead of the XMP 2.0 2666 timing set. I suspect your memory is already running at 2666 so this may be just a reporting...
